Analysis of the osteolysis around the porous coated femoral stem of bipolar hemiarthroplasty for non-traumatic osteonecrosis of the femoral head

Abstract
Purpose: The purpose of this study was to analyze the factors influencing the osteolysis of bipolar hemiarthroplasty, using a porous coated femoral stem in non-traumatic osteonecrosis of the femoral head.

Materials and methods: Between January 1989 and December 1996, 38 hips in 31 patients, with Ficat stage III of non-traumatic osteonecrosis of the femoral head, were treated with bipolar hemiarthroplasty, using porous coated Harris-Galante type I or Multilock femoral stems and a biarticular cup (Zimmer, Warsaw Indiana, USA). The average follow-up was 8.5 years, ranging from 5 to 11 years. The factors influencing the development of osteolysis around the femoral stem of the bipolar hemiarthroplasty were statistically analyzed.

Results: The over all incidence of periprosthetic osteolysis was 39.5% (15 hips). The type of stem and the stem stability are significantly associated with the presence of periprosthetic osteolysis. In contrast, the age, sex or body weight of the patient, etiology of the osteonecrosis, size of the bipolar cup and the femoral stem, wear of the articular cartilage of the acetabulum and the Harris hip score had no correlation with the periprosthetic osteolysis.

Conclusion: Periprosthetic osteolysis of a bipolar hemiarthroplasty, with a porous coated femoral stem, was influenced by the type of femoral stem and its stability.
